Finally the third position of the face, is the lower face and neck line. And this seems to be an area almost every patient would like to improve if they could.
The question is, "is there a bad enough problem there to want surgery?" When there is enough sagging of the jowl line and the neck line, we use an incision around the ears. this usually extends from the temple, in front of the ear, in the crease behind the ear, and into the scalp behind the ear.
There's many different operations that have sought to reduce the extensiveness of this scar. So called minimally invasive or mini face lifts. However, the truth is, is that the incisions in the scalp are the least noticeable, and sometimes a patient would cut themselves short, if we limit the operation to scars only around the ear.
When we do this incision, we want to expose the deeper structures of the face, the SMAS, which stands for, The superficial muscular and aponeurotic system , which is a big, fancy, anatomic term, . . . it really is the fatty tissue, which is quilted together with connective tissue and gives our face its shape.
This is the structure that we want to pull and lift, this is where we want to put the tension, not on the skin. That leaves a pulled, taut, or wind tunneled look. And that can be totally avoided if the underlined structures are pulled tight or suture.
